Keep in mind that your level of charge code is determined by your provider at the time of service. The practice performs services in both the “office” and “hospital” setting. Our pricing may change from time-to-time (updated annually) and remember these are estimated charges and there could be more than one code charged for a visit, i.e., an office service and a procedure performed during the same visit. The actual charges for health care services are dependent on the circumstances at the time the service is rendered.
